Asset mapping
An assessment process for identifying existing resources and other assets in the community that can serve as potential partners in providing bidirectional and meaningful linkages to care. When work-related stress becomes so overwhelming that it leads to physical and emotional exhaustion, cynicism, and depersonalization. In the workplace, diversity refers to creating a culture of inclusion that respects different social identities, lived experiences, and perspectives. Diversity serves as a catalyst for system change, ensuring that marginalized, underrepresented people have the voice, tools, and power to lead in all levels of the healthcare system. Preventable differences in health conditions and mortality that negatively affect groups of people who have experienced systemic discrimination and bias based on race, ethnicity, sexual orientation, gender identity, disability and additional characteristics historically associated with discrimination and marginalization. The process of removing economic, social, and other structural barriers while creating and strengthening fair and just practices and structures so that all people can attain optimal health and wellness. A problem-solving process consisting of three phases: inspiration, i.e., using interviewing and observation to ground solutions in lived experiences; ideation, i.e., identifying, testing, and refining solutions; and implementation, i.e., building partnerships and scaling-up the solution. The knowledge a person gains from direct, first-hand involvement with an event, or from living as a person with a particular identity. For example, a person in recovery from opioid use has lived experience with an opioid use disorder. The process of increasing and improving patient access to a full array of treatment, care, and supportive services through robust and sustainable partnerships among the health center and partners in the community, including the medical community, local community organizations, and public health agencies. Together, the partners agree on shared goals and values to support all patients and clients, and to advance health equity.
